Our main office is located at Fern Hill Medical Campus, Building B, Suite 300, 915 Old Fern Hill Road, West Chester, PA 19380.

Our satellite offices are in Kennett Square at 127 West Street Road, Suite 102, Kennett Square, PA 19348, and at The Commons at Oaklands, 736 West Lincoln Highway, Exton, PA 19341.

To schedule an appointment call 610-431-3122. Please be prepared to provide the patient’s name, address, phone number, birth date, type of insurance, reason for appointment and referring physician, if applicable.

Please arrange to arrive fifteen minutes prior to your appointment time so that any paper work may be processed before your visit with the doctor. We will make every effort for you to see the doctor at your scheduled time. To expedite your visit, it is important that you complete the patient registration form/medical history form. For your convenience, you may download a copy of the Patient Registration form to complete prior to your visit. Please be sure that you bring your current health insurance cards and your co-payment, and a referral from your primary medical doctor if applicable.

Emergencies: If you are experiencing a medical emergency, please call 911. If you are experiencing an urgent problem but not a true emergency, please call 610-431-3122. Our answering service will page the physician on-call, who will return your call as quickly as possible.

Cancellations: Keeping your scheduled appointment is important. However, if it is necessary to reschedule your appointment, please call 610-431-3122 at least 24 hours in advance so that we can meet the needs of other patients during your cancelled appointment time. If you need to re-schedule your endoscopic procedure, please let us know at least one week in advance whenever possible.

Prescription Refills: There are three easy ways to obtain a prescription refill. You may
(1) Ask your pharmacy to fax a refill authorization - Fax # 610-431-4799
(2) Call 610-431-3122 and talk with a staff member
(3) Leave a message on our Rx voice mail box by calling 610- 431-3122 ext 2

Insurance: We participate in Medicare, Medicaid, Blue Cross, Major insurance plans and most HMOs and PPOs. Many health plans require a referral from a primary care physician. Please check with your insurance carrier when calling for an appointment.

Hospital affiliation: We are on the staff at Chester County Hospital in West Chester, PA. We provide in-hospital consultations 24 hours a day, 7 days a week as requested by primary care physicians or the emergency department.
